Rates
Rates for evaluations are set at $250 for the parent/caregiver intake session and $200 per subsequent hour of clinical work, scoring, interpreting, feedback, and report writing. Total evaluation fees may range from $500 for brief psychological testing up to $3500 for comprehensive evaluations.
Insurance
We do not bill insurance at this time. Depending on your out-of-network coverage, you may be able to receive partial reimbursement from your insurance provider. You will be provided a superbill (invoice), which you may submit to your insurance provider for possible reimbursement. Questions to ask your insurance provider:
Do I have Out-of-Network coverage?
What are my Out-of-Network benefits?
What is my deductible?
After I meet my deductible, what percentage of the visit cost will I be reimbursed for?
Will Telehealth services (with with location 2 and modifier GT/95) be reimbursed on my plan?
You may provide the following procedure codes to your insurance company ahead of time to determine if they provide coverage for out-of-network providers:
- 90791 (intake appointment)
- 96136 and 96137 (psychological test administration and scoring codes)
- 96130 and 96131 (psychological evaluation services, including interpretation, report writing, and feedback appointment)
- 96132 and 96133 (neuropsychological evaluation services, including interpretation, report writing, and feedback appointment)
